Elevare Balance provides personalized medical support for adults who have reached a stable phase of treatment and want help maintaining their progress, managing medication appropriately, and responding early when their needs change.
Is Weight-Maintenance Care Right for You?
The Medical Weight Maintenance Program may be appropriate if you:
• Have reached an individualized goal or stable weight range
• Are currently using anti-obesity medication and need ongoing medical management
• Are transferring from another medical weight-management practice
• Are considering reducing or discontinuing medication
• Have concerns about appetite changes or weight regain
• Would benefit from continued monitoring, education, and accountability
An initial consultation is required so Elevare can review your medical history, current treatment, medication source and dose, health goals, and available laboratory results.
What Your Membership Includes
• Medical follow-up approximately every 8–12 weeks when clinically stable
• Prescription and routine refill management
• Monitoring of weight, appetite, treatment response, and side effects
• Nutrition, physical activity, and lean-mass-preservation support
• Secure portal messaging between visits
• Review of relevant laboratory results
• Coordination with your primary care clinician, with your authorization
Your Maintenance Plan Is Individualized
Maintenance does not automatically mean stopping medication or reducing it to a particular dose. Obesity is a chronic medical condition, and some people benefit from ongoing treatment.
Your plan may include continuing medication, adjusting medication, or monitoring your progress without medication. Decisions are made together based on your health, treatment response, side effects, preferences, and medication access.
No particular medication, prescription, dose, insurance coverage, or treatment outcome is guaranteed.
Medical Weight Maintenance Membership
$99 per month
Medication, laboratory testing, imaging, and services provided outside Elevare Balance are not included.
If your needs change and you require active medication titration, more frequent visits, or additional weight-loss support, we may recommend transitioning to Elevare’s standard Medical Weight Management Membership.
