Comprehensive Consultation
Meet one-on-one with a medical provider to review your health history, medications, goals, previous weight loss experiences, and current care. Together, we’ll determine which treatment options may be appropriate for you.
Personalized Treatment Plan
Receive an individualized plan that may include medication when appropriate, practical nutrition and lifestyle guidance, realistic goals, and education designed around your health needs and daily life.
Connected Follow-Up Care
Through regular follow-ups, medication monitoring, lab review, and education, we help keep your care safe and supported- communicating with your healthcare team when needed and with your permission.
You Lost the Weight. Now Let’s Protect Your Progress
Elevare’s Medical Weight Maintenance Program provides ongoing medical follow-up, medication management, and personalized support for adults who have reached a stable phase of treatment. Membership is $99 per month. An initial consultation is required to determine whether maintenance care is clinically appropriate.
For Primary Care Clinicians
Elevare Balance provides focused medical weight-management care designed to complement primary care. We offer patient education, medication management, and longitudinal follow-up while keeping the referring clinician at the center of the patient’s overall healthcare.
