  3. Before the consultation, the patient completes a comprehensive health history and appropriate consent forms.
  4. The patient receives an individualized clinical evaluation and treatment recommendations.
  5. With the patient’s authorization, we communicate relevant recommendations, medication changes, and clinical concerns to the referring clinician.
